Physical Therapist for Pediatric and Adult Patients (Home Health)

Position: Licensed Physical Therapist (Pediatric and Adult)

Location: San Antonio

Schedule: Full-Time

Key Responsibilities:

Conduct in-home assessments and develop individualized treatment plans for patients.
Implement interventions to improve independence and educate families on integrating therapy into daily routines.
Collaborate with a multidisciplinary team to provide holistic care and adjust plans as needed.
Document patient interactions and maintain accurate records in an electronic system.
Complete 25-35 visits per week while maintaining patient confidentiality.

Qualifications:

Valid Physical Therapist (PT) license required
Experience in home health or skilled nursing required
