The Field Based Community Health Worker (Ambassador) is a pivotal member of the Landmark Interdisciplinary Team. This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients’ and caregivers’ trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.

This position is full – time, Monday – Friday. Employees are required to work any shift between the hours of 8:00 AM – 4:30 PM. Our office is located at 395 OYSTER POINT BLVD, STE 512, SOUTH SAN FRANCISCO, CA, 94080.

Primary Responsibilities:

  • Conduct home visits and / or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to:
    • Resource referral and connection
    • Quality measure documentation, support, and follow up
    • Telemedicine facilitation and set up
    • Collect information regarding patient conditions using standardized checklists and templates.
    • Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction
    • Actively participate in weekly interdisciplinary team meetings

Required Qualifications:

  • High School Diploma / GED (or higher)
  • 3+ years of experience in any of the followings job categories: Home Health Aide; Certified Nursing Assistant; Certified Medical Assistant; Medication Aide; Health Coach; Health Navigator OR Healthcare Technician; OR other related healthcare experience dealing with patients
  • Ability to conduct independent patient home visits OR have prior experience with making home visits to patients
  • Experience with using Microsoft Office products, including Microsoft Word (draft, save, and share documents), Microsoft Excel (create, save, share, and use basic formulas), Microsoft PowerPoint (create, save, and share presentations), and Microsoft Outlook (draft emails and manage calendars)
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy
  • Ability to travel up to 75%
  • Flexible to conduct patient visits to Sacramento, CA for at least 1 – 2 times per month
  • Reside within a commutable distance of San Francisco, CA
  • Ability to work any shift between the hours of 8:00 AM – 4:30 PM from Monday – Friday
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:

  • Experience with geriatric OR primary care experience, OR experience in institutional settings of care such as long – term care / skilled nursing facilities
  • Social Worker experience
  • Knowledge of medical terminology

  • Superb written and oral communication skills, and comfort level with technology

Physical and Work Environment:

  • Ability to travel frequently by car
  • Regular requirement to talk and listen
  • Occasional requirement to lift and / or move up to 10 pounds

  • Occasional requirement to stand; walk and reach with hands and arms
  • Vision abilities include close vision, peripheral vision, depth perception and ability to adjust focus

California Residents Only: The salary range for California residents is $18.80 – $36.78.
