Lymphedema/Parkinsons Therapist
We are seeking a dedicated and patient-focused Lymphedema/Parkinson's Therapist to provide therapy services in patients' homes in the Punta Gorda, Port Charlotte, North Port, and Englewood areas. The ideal candidate will deliver effective, client-centered care that empowers individuals to carry out daily tasks confidently and independently. A holistic approach and the ability to adapt the environment to meet the unique needs of each patient are essential.
General Accountabilities
**Key Responsibilities: **
- Assess patients' abilities to safely perform independent living skills.
- Design and implement personalized care plans, determining the type and frequency of treatment.
- Utilize appropriate modalities to execute treatment plans effectively.
- Document patient progress and adjust care plans as needed.
- Assist with patient discharge from therapy services, providing guidance on adaptive equipment and support programs.
- Educate and support family members and caregivers in the therapeutic process.
- Observe and report on the progress of both long and short-term treatment goals.
- Collect data and maintain thorough documentation of processes followed.
- Conduct regular follow-ups with clients to ensure continuity of care.
Job Qualifications
- Proven experience as a Certified Lymphedema Therapist.
- In-depth knowledge of lymphedema therapy principles and practices.
- Hands-on experience in measuring patients and understanding various therapeutic garments.
- Ability to effectively manage patients with diverse personalities.
- Commitment to ongoing professional development and staying current with treatment practices.
- Professional demeanor with excellent interpersonal communication skills.
- Valid state license to practice as a Physical Therapist, Occupational Therapist, or Occupational Therapy Assistant.
- Basic Life Support (BLS) certification.
- Professional Liability Insurance.
