In our Full Service Partnership located in Pasadena (www.socialmodel.com), responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the participant’s health and human services needs.
Essential Duties and Responsibilities
- Role model appropriate healthy and ethical behaviors in all interactions, including HIPAA and Confidentiality standards.
- Maintain a primary caseload of residents/participants, assisting and advocating for them in meeting their unique needs within the program structure.
- Develop a treatment plan for each resident/participant on the caseload and evaluate their progress in meeting the goals and objectives within the treatment plan.
- Update the individual treatment plans, as appropriate, in a timely manner.
- Document the services provided according to standards set by the Agency and applicable County, State, and Federal regulations. Documentation should be clear, concise, and completed in a timely manner.
- If determined by the Program Director, maintain and update program curricula.
- Assist with resident’s/participant’s benefits (e.g. DPSS, SSI, Medi-Cal) application(s).
- Familiarity and adherence to all regulatory expectations and ethical standards and Agency policies and procedures, including the Agency’s Core Beliefs.
- Maintain constructive relationships with family members, community members and organizations, human services agencies, and other professionals.
Required credential: Registered/Certified Substance Abuse Counselor OR licensed LMFT, LCSW or pre-licensed
