Responsibilities

Conducts the initial comprehensive assessment of patients upon admission to ensure a thorough understanding of their medical history, current health status, and specific needs.
Engages in ongoing and periodic assessments to consistently evaluate the evolving needs of each patient.
Monitors and evaluates the effectiveness of services provided, ensuring that they meet established standards and result in satisfactory patient care.
Tracks the progress of each patient towards their individual goals, utilizing measurable indicators to demonstrate improvement and areas needing attention.
Oversees the critical transitions for patients, carefully assessing all discharge planning needs to ensure a seamless transfer to the next phase of care, whether at home or in another facility.

Requirements

BSN required
New York RN license/registration.
One year of Hospital Care Management experience
Care Management Certification
PRI Certification
