Responsibilities
- Provide comprehensive nursing care to hospice patients in their homes.
- Develop, implement, and evaluate individualized care plans in collaboration with the multidisciplinary team.
- Administer medications and treatments as prescribed, ensuring proper documentation.
- Monitor and assess patient conditions, adjusting care plans as necessary based on observed changes.
- Educate patients and families on disease processes, pain management, and care strategies.
- Offer emotional support and counseling to patients and families facing end-of-life issues.
- Coordinate with physicians, social workers, and chaplains to meet the holistic needs of patients.
Requirements
- Current and valid nursing license (RN) in the state of practice.
- Bachelor's degree in Nursing (BSN) preferred.
- Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care.
- Strong knowledge of pain management and symptom control in hospice care.
- Excellent communication and interpersonal skills to work effectively with patients and families.
- Ability to work collaboratively in a multidisciplinary team environment.
