Job Summary
The Supv, Eligibility & Screening is responsible for providing day to day supervision and leadership to the Eligibility Screeners. The Supervisor assists with training, support, guidance, and the on-going development of their Eligibility Screening team and acts in a consulting capacity to other key stakeholders.
Job Duties

Support, train, and supervise Eligibility Screeners, assist in coordinating the hiring and training for new Eligibility Screeners, conduct check-ins with team members, provide feedback and make disciplinary decisions in regards to Eligibility Screeners, approve PTO and timesheets and ensure ongoing compliance and quality of Eligibility Screeners’ work.

Assist in problem solving and decision making related to screens such as participant or member-related issues, adverse eligibility findings, timing of screens, coordinating screens with moves and discharges or other issues that may arise.

Attend meetings to act as a subject matter expert and act in a consulting capacity to key stakeholders in regards to the Long Term Care Functional Screen (LTC-FS) program and also attend and facilitate Fair Hearings related to functional ineligibility.

Complete the Adult LTC-FS as required and outlined in the Wisconsin Adult LTC-FS Instructions. This includes completing collateral contacts to verify screen findings with internal and external stakeholders, and verifying diagnosis information with physicians and or SSA.

Supervisors must comply with documentation and program regulations, guidelines and meet screening quota.

Participate in and assist with All Screener Meetings, quizzes and testing.

Collaborate on the development, process improvement and ongoing quality management of services and related materials/products.

As requested, provide data reports and complete other duties as assigned
Job Qualifications
REQUIRED EDUCATION:
Bachelor of Arts or Science degree or more advanced degree in a health or human services related field (e.g. social work, psychology) or graduate from an Accredited School of Nursing.
REQUIRED EXPERIENCE/KNOWLEDGE, SKILLS & ABILITIES:

1+ years of experience working with one of the target groups such as adults with physical or intellectual disabilities or older adults.

Must be currently certified to conduct/administer the Adult Long Term Care Functional Screens or pass the Wisconsin Adult Long-term Care Functional Screen certification modules (80% or higher on each module) within first week of hire and maintain certification as a screener.

Past experience supervising or mentoring employees

Demonstrated computer and software skills required; proficiency with Microsoft Office Suite and be able to work in and navigate through databases.

Strong writing skills and attention to detail.

The ability to work cooperatively as part of a team.

The ability to work autonomously.

Ability to remain flexible in the work environment and willing and able to adapt to changing organizational needs.

Travel Requirements 15%
REQUIRED LICENSE, CERTIFICATION, ASSOCIATION:
-
Must possess a valid driver’s license, maintain adequate auto insurance for job-related travel and ability to travel throughout Wisconsin.

If a graduate from an Accredited School of Nursing, must have an active, unrestricted Wisconsin Registered Nursing (RN) licence in good standing.

Pay Range: $49,930 - $97,363 / ANNUAL
