The role you’ll contribute:
The Patient Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
GENERAL SUMMARY:
Coordinates access to health-related services by assisting clients with utilizing available resources
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
· Identifies potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.
· Provides informal counseling, education, and social support to clients. Assists clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
· Develops and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
· Maintains accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
Qualifications
EDUCATION AND EXPERIENCE REQUIRED:
·Highschool Diploma is Required; Associate's and/or bachelor's preferred.
· Bariatric Experience is a PLUS!
