The Transformation Consultant reports directly to the Director of Provider Transformation. The Transformation Consultant is responsible for the following:

Assist with the design of the enterprise wide and/or cross-functional solutions to address Joint Venture (JV) business needs. This work results in the implementation of initiatives that impact JV provider strategy enabling the VBC/Network teams to be competitive in the marketplace and effectively serve its customers.

Partners with business owners to optimize the achievement of strategic business objectives, to include profitable growth and improved organizational effectiveness.

Responsible and accountable for day to day tactical follow through of the transformation road map including implementation and tracking of programs within the JV that will improve total medical cost, membership growth and achieve the Triple Aim.

Anticipates implementation challenges and develops solutions to overcome change resistance from Client thereby mitigating delays or challenges in attaining roadmap implementation objectives and timelines.

Develops transformational projects to position assigned clients for sustainable population health effectiveness across their full continuum of care delivery providers.

Generally, Supports between 8-10 JVs.

Involved in initiative planning, ongoing client relationship management and customer satisfaction for assigned initiatives that impact the delivery of products and services to the internal and external environment.

Collaborates with external vendors, external consultants and Aetna team members as required in support of initiatives.

Accountable for transferring knowledge, best practices (external and internal), methodology and tools to client organizations.

Participates as a team member, may be responsible for concurrent initiatives or act as team lead for less complex projects based on resource requirements.

Utilizes project management principles and processes to promote successful population health management outcomes.

Provides support to assigned clinical transformation program implementation efforts.

Provides supports in coordinating project activities, meetings, and status reporting.

Collaborates with Informatics team to make sure all required reporting is provided in a timely manner and per scheduled client meetings.

Background/Experience Desired

3-5 years of experience in health care provider or payor environment project management.

Pay Range
The typical pay range for this role is:
$82,940.00 - $182,549.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. This position also includes an award target in the company’s equity award program.
