Position Summary
We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join our Case
Management team. This opportunity offers a competitive salary and full benefits. Our organization promotes autonomy through a Monday -
Friday working schedule, paid holidays, and flexibility as you coordinate the care of your members. Case Management Coordinator is
responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with
members to evaluate the medical needs of the member to facilitate the member's overall wellness. Case Management Coordinator will
effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address
issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through
integration. Case Management Coordinators will determine appropriate services and supports due to member's health needs; including but
not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.

Candidates must be located in Temple Terrace, Tampa, FL or USF area or immediately surrounding.

Candidate must also be fluent bilingual in Spanish (reading, writing, and speaking).

Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough county and surrounding areas in FL

Schedule is Monday-Friday, 8:00am-5:00pm, standardbusiness hours
Required Qualifications

Bi-lingual - Fluent English and Spanish (reading, writing, and speaking)

1 + year of experience in behavioral health or a long-term care setting

Effective communication skills, both verbal and written

Computer proficiency in Microsoft Word, Excel, and Outlook

Managed care experience

Education
- Bachelor's Degree in Social Work or related field

Pay Range
The typical pay range for this role is:
$21.10 - $36.78
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
