Job Title: Part-Time Physical Therapist (Home Health)

Locations: Tarrant, Denton, Dallas, Collin counties

Position Overview: We are seeking a skilled and compassionate Physical Therapist to join our team on a part-time basis. As a Home Health Physical Therapist, you will play a vital role in improving the quality of life for our patients by providing specialized rehabilitative care in the comfort of their homes. This position offers the opportunity to work with a diverse patient population, collaborate with interdisciplinary teams, and make a meaningful impact in the community.

Responsibilities:

Conduct comprehensive evaluations to assess patients' physical abilities and limitations.
Develop individualized treatment plans tailored to meet patients' specific needs and goals.
Provide skilled physical therapy services, including therapeutic exercises, manual therapy techniques, gait training, and functional mobility training.
Educate patients and their caregivers on therapeutic exercises, safety measures, and techniques for maximizing independence.
Regularly reassess patients' progress and adjust treatment plans as necessary.
Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment outcomes.
Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated and integrated care.
Adhere to all regulatory requirements and ethical standards governing physical therapy practice.

Qualifications:

Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
Valid license to practice as a Physical Therapist in the state of Texas.
Minimum of one year of clinical experience as a Physical Therapist, preferably in a home health or outpatient setting.
Strong clinical skills in conducting evaluations, developing treatment plans, and implementing evidence-based interventions.
Excellent interpersonal and communication skills with the ability to establish rapport with patients and their families.
Ability to work independently and collaboratively within a multidisciplinary team.
Reliable transportation and willingness to travel to patients' homes in the South Dallas, Lancaster, DeSoto, Cedar Hill, and Duncanville areas.
CPR certification preferred.
