Job Title: Part-Time Physical Therapist (Home Health)

Position Overview: We are seeking a skilled and compassionate Physical Therapist to join our team on a part-time basis. As a Home Health Physical Therapist, you will play a vital role in improving the quality of life for our patients by providing specialized rehabilitative care in the comfort of their homes. This position offers the opportunity to work with a diverse patient population, collaborate with interdisciplinary teams, and make a meaningful impact in the community.
Responsibilities:
- Conduct comprehensive evaluations to assess patients' physical abilities and limitations.
- Develop individualized treatment plans tailored to meet patients' specific needs and goals.
- Provide skilled physical therapy services, including therapeutic exercises, manual therapy techniques, gait training, and functional mobility training.
- Educate patients and their caregivers on therapeutic exercises, safety measures, and techniques for maximizing independence.
- Regularly reassess patients' progress and adjust treatment plans as necessary.
- Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment outcomes.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated and integrated care.
- Adhere to all regulatory requirements and ethical standards governing physical therapy practice.
Qualifications:
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Valid license to practice as a Physical Therapist in the state of Texas.
- Minimum of one year of clinical experience as a Physical Therapist, preferably in a home health or outpatient setting.
- Strong clinical skills in conducting evaluations, developing treatment plans, and implementing evidence-based interventions.
- Excellent interpersonal and communication skills with the ability to establish rapport with patients and their families.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Reliable transportation and willingness to travel to patients' homes in the South Dallas, Lancaster, DeSoto, Cedar Hill, and Duncanville areas.
- CPR certification preferred.
