Occupational Therapists
As an Occupational Therapist at our agency, you will provide home-based therapy services under physician orders, conducting comprehensive assessments to evaluate the patient's functional level and needs. You will develop and implement individualized treatment plans, guide patients in therapeutic activities, and teach them how to use assistive devices like wheelchairs, braces, and splints. You will collaborate with physicians and the interdisciplinary team, monitor patient progress, and adjust care plans as needed to enhance the patient's independence and physical functioning. Additionally, you will supervise Occupational Therapy Assistants and Certified Home Health Aides, participate in discharge planning, and engage in continuous professional development to stay current with best practices in the field.
Qualifications:
-Must be a graduate of an Occupational Therapy School
-Must be licensed in state of NJ
-Preferably, should be a member of AOTA
Occupational Therapy Assistant
Under the supervision of the Occupational Therapist (OT), you will provide home-based occupational therapy services, assist in evaluating and developing individualized care plans, and support patients in therapeutic activities aimed at improving their physical and mental functioning. You will guide patients in the use of self-help devices, assist with exercises, and monitor their progress while documenting daily activities and communicating with the OT and physician about any changes in the patient's condition. Additionally, you will instruct family members and other healthcare team members on appropriate therapy techniques and ensure coordination of visits with the Director of Nursing. Your role also includes participating in discharge planning, preparing progress notes, and engaging in ongoing professional development to enhance your skills and knowledge.
Qualifications:
-Must be a graduate of an approved Occupational Therapy Assistants school
-Must be licensed in state of NJ
-Must have a minimum of one-year experience in OTA
Physical Therapist
As a Physical Therapist at our agency, you will provide skilled, home-based physical therapy services in accordance with physician orders, conducting comprehensive assessments and developing personalized treatment plans. You will utilize various physical therapy techniques such as exercise, massage, and modalities like heat and light to relieve pain and restore function, while instructing patients and families on proper use of assistive devices.Your responsibilities also include participating in case conferences, discharge planning, and ensuring all documentation and quality assurance standards are met.
Qualifications:
-Must be licensed in state of NJ
-Must be a graduate of a Physical Therapy school
Physical Therapy Assistants
Under the direction of a Registered Physical Therapist (PT), you will provide home-based physical therapy services, assist in evaluating and developing rehabilitative care plans, and perform therapeutic exercises for patients. You will also document patient progress, report findings to the PT and physician, and educate patients and their families on exercise programs and proper use of assistive devices. Additionally, you will collaborate with the interdisciplinary team, assist in discharge planning, and engage in continuous professional development to enhance your skills and knowledge.
Qualifications:
-Must be a graduate of physical therapy assistants school
-Must be licensed in state of NJ
-Must have a minimum of one year experience as PTA
Speech Pathologist
You will provide skilled speech pathology services to patients in their homes, following physician orders, including initial and ongoing assessments of their needs and the development of treatment plans in collaboration with the physician. Your responsibilities include instructing patients and their families in speech therapy techniques, monitoring progress, and ensuring proper coordination with other healthcare team members. Additionally, you will document treatments, evaluate the effectiveness of interventions, and update care plans as necessary while participating in discharge planning and assisting with referrals for outpatient services. You will also engage in professional development, attend meetings, and ensure that the physician’s orders and care plans are properly followed and revised as needed.
Qualifications:
-A Masters Degree in Speech Language Pathology
-Must be licensed in the state of NJ
-Preferably a member of American Speech-Language and Hearing Association
