The Memory Care Nurse is responsible for the overall coordination of care and life enrichment experience in the memory care unit. The nurse will provide resident care coordination, medication management/supervision and clinical care of the residents in Memory Care. The Memory Care Nurse directly supervises and mentors the direct care staff to provide resident care, facilitate resident activities, medication management and clinical care of the residents. The Memory Care Nurse will be responsible for leading a dementia program that will enrich the lives of the residents in Memory Care.
Job Responsibilities:
- Responsible for the initial prescreen, medication check on admission, physician orders/ ensuring the DME is completed correctly.
- Responsible for the initial, annual, and significant change assessment/support plans and obtaining DME’s as needed.
- Responsible to attend Support plan Meetings within 30 days of admission, annually and significant changes of the resident.
- Responsible for executing physician orders with an understanding of why the orders were issued and with a knowledge of their desired or possible adverse effects.
- Assist the physician in obtaining accurate medical histories and with communicating to the physician as needed.
- Records and reports pertinent observations of residents’ responses to medical, nursing and therapy to the physician and POA.
- Effective communicating with resident physicians with changes by preparing an appropriate assessment
- Participates with the Administrator and or PC team in planning directing and coordinating resident care, staff assignments and supervision.
- In absence of the PC Administrator, oversees performance and behaviors of clinical staff to ensure that all policies and procedures and clinical standards are upheld. Reports any violation of policy or procedure to the Administrator.
- Reports complaints and concerns made by the resident, families or staff and make a written/oral report s to the appropriate regulatory agency when necessary.
- Participates in assisting the PC team to train and educate direct care staff in dementia care and clinical skills.
- Be either a medication observer/Medication Train the Trainer
- Prepares residents for medical tests by communicating with the direct care staff and resident necessary instructions.
- Regularly communicates information to the residents, families and POA when needed.
- Complete all discharge instructions and communicate with the resident and POA.
- Audit clinical systems and or medical records to ensure compliance with all clinical systems (wound documentation weekly, vital signs monthly or when ordered by the PCP, weights, EMAR) assessment completion.
- Meet and greet all new admissions on the shift and do a physical assessment (skin check and physical assessment.)
- Assist in the supervision and orientation of new direct care staff, memory car activity staff and nursing staff.
- Lead investigations for injuries of unknown origin and or abuse allegations as needed.
- Complete Regulatory reports to DHS/AAA in the absence of the Administrator.
- Follow up with staff to ensure that fall investigations are initiated and complete a post fall assessment in the electronic record.
- Review and complete all incident reports on your unit assignment within a week of the incident.
- Rotating On call 24/7 except when on vacation/PTO and MOD coverage monthly.
- Attend weekly Collaboration meetings.
- Attend daily stand-up meetings.
- Holds regular meetings with the Team Members.
- Attend monthly Personal Care QAPI meetings
- Responsible for evaluating Team Members Performance in the unit and completing annual reviews or necessary Performance Reviews.
- Maintains the confidentiality of all Resident Care Information.
Conforms with the requirements of the compliance program by understanding and adhering to Chandler Halls Compliance Program and Policies as they appear in the Code of Conduct and other compliance policies. Annually acknowledges review of the Code of Conduct by signature and annually participates in corporate compliance education. Adheres to the policies and procedures covering Privacy and Security in compliance with HIPAA regulations.
Competencies:
Nursing license in maintained
CPR certified
Maintains 12 hours of required DHS training- additional 6 hours of Dementia training annually if you work in the secured unit
Education and Experience
Nursing Diploma or Degree
One year experience in a supervisory role
