**Job Title:** Care Manager, RN (USC Only)
**Location:** New Castle County, DE (must reside in Delaware)
**Responsibilities:**
- Travel to members’ homes (and occasionally hospitals/motels) to conduct in-person assessments and follow-up via phone.
- Coordinate care for members with chronic health issues and complex needs, ensuring safe and cost-effective service delivery.
- Facilitate transitions between care settings (e.g., hospital to home, nursing facility to community).
- Educate members and caregivers about healthcare needs, available benefits, and resources.
- Develop and implement care plans with members/caregivers, addressing physical, behavioral, and social service needs.
- Coordinate care with interdisciplinary teams and community resources, including disease management and behavioral health programs.
- Monitor and evaluate the effectiveness of care plans and services, making adjustments as needed.
- Document all case management services in the electronic health record, adhering to privacy and regulatory standards.
**Required Qualifications:**
- Registered Nurse (RN) in DE with case management experience.
- Experience in assessments, care plan development, and coordinating with healthcare providers.
- Proficiency in Microsoft Office and electronic documentation systems.
- Reliable transportation and ability to travel within assigned territory.
- Experience in geriatrics, home health, and social determinants of health.
