As a Physical Therapist/Occupational Therapist at Love and Mercy Home Healthcare, you will play a critical role in helping patients recover from injury, surgery, illness, or chronic conditions in a home setting. You will assess patients' physical and functional abilities, develop personalized treatment plans, and help patients regain their independence through tailored rehabilitation programs. Key Responsibilities:
- Evaluation & Assessment: Perform initial assessments of patients’ physical, functional, and occupational abilities, creating personalized treatment plans based on these evaluations.
- Treatment Implementation: Develop and implement individualized physical therapy and/or occupational therapy treatment programs to improve mobility, strength, coordination, and daily living skills.
- Patient Education: Educate patients and their families on therapy techniques, exercise routines, and strategies to promote recovery and prevent injury.
- Progress Monitoring: Monitor and document patient progress, making necessary adjustments to treatment plans to ensure continuous improvement.
- Collaboration: Work with other healthcare professionals (such as nurses, physicians, and speech therapists) to ensure holistic care and a coordinated approach for each patient.
- Care Coordination: Provide updates and communicate regularly with the care team regarding patient progress, treatment changes, and concerns.
- Patient Support: Offer emotional and motivational support to patients and families throughout the rehabilitation process.
- Travel: Travel to patient homes within the assigned service areas of Dorchester, Roxbury, South Shore, Brockton, Randolph, and nearby areas.
Qualifications:
- Licensed Physical Therapist (PT) and/or Licensed Occupational Therapist (OT) in Massachusetts.
- Experience: At least 1-2 years of experience as a PT/OT, preferably in home healthcare or outpatient settings.
- Skills: Strong clinical knowledge of physical and occupational therapy techniques, patient care, and rehabilitation programs.
- Independence: Ability to assess patient needs, develop individualized care plans, and work autonomously with minimal supervision.
- Communication: Excellent interpersonal and communication skills to collaborate effectively with patients, families, and healthcare teams.
- Travel: Reliable transportation and the ability to travel to patient homes within the service areas.
- Certifications: CPR and First Aid certification preferred.
- Compassion: A patient-centered, compassionate approach to care with a focus on improving the quality of life for each patient.
