Job Summary:
We are seeking a detail-oriented and experienced Remote Nurse to assist in transforming shorthand physician notes into comprehensive, compliant progress notes using Practice Fusion EMR. The ideal candidate will have a strong understanding of medical terminology, SOAP note formatting, and insurance documentation requirements. This role requires excellent communication skills, the ability to work independently, and a commitment to accuracy and efficiency.
Key Responsibilities:
- Documentation Transformation:
- Convert shorthand physician notes into detailed, compliant progress notes in Practice Fusion EMR.
- Ensure all notes follow the SOAP (Subjective, Objective, Assessment, Plan) format and meet insurance company requirements.
Chart Review and Completion: - Review patient charts for completeness and accuracy.
- Add relevant details, including history of present illness (HPI), review of systems (ROS), physical exam findings, assessment, and plan.
Compliance and Billing Support: - Ensure all documentation meets regulatory and billing standards (e.g., ICD-10 coding, CPT coding, and Medicare/Medicaid requirements).
- Collaborate with providers to clarify any missing or unclear information.
EMR Management: - Navigate and utilize Practice Fusion EMR efficiently to input and update patient records.
- Maintain organized and up-to-date patient files.
Communication and Collaboration: - Work closely with the physician to ensure accurate and timely documentation.
- Provide feedback and suggestions to improve documentation processes.
Quality Assurance: - Perform regular audits of progress notes to ensure compliance with organizational and regulatory standards.
- Stay updated on changes in documentation requirements and EMR functionality.

Qualifications:
- Education:
- Bachelor’s degree in Nursing (RN)
- Experience:
- Minimum 1-2 years of experience in medical documentation, scribing, or clinical support.
- Proficiency with Practice Fusion EMR or similar electronic medical record systems.
- Familiarity with SOAP note formatting and insurance documentation requirements.
- Skills:
- Strong knowledge of medical terminology, anatomy, and physiology.
