SearchPlus HR is excited to announce an opening for a Clinical Pharmacist to join our innovative healthcare team. In this role, you will be instrumental in enhancing patient care through effective medication management and expert pharmaceutical guidance.
Your primary responsibilities will include assessing medication orders, providing drug information to healthcare providers, and educating patients about their prescriptions. You will collaborate with a multidisciplinary team to design personalized medication plans and monitor patient responses, ensuring the best therapeutic outcomes are achieved.
Key Responsibilities
- Review and evaluate medication orders for appropriateness and potential interactions
- Counsel patients on medication usage, adherence, side effects, and lifestyle modifications
- Participate in multidisciplinary rounds to provide pharmaceutical expertise
- Conduct medication therapy management and therapeutic drug monitoring
- Document all clinical activities and recommendations thoroughly
- Stay informed about updates in drug therapies and pharmacotherapeutics
- Engage in quality assurance and improvement initiatives within the pharmacy department
Requirements
- Doctor of Pharmacy (PharmD) from an accredited institution
- Current pharmacy licensure in the relevant state
- Previous clinical pharmacy experience is preferred
- In-depth knowledge of pharmacology and pharmacotherapy principles
- Exceptional communication and interpersonal skills
- Critical thinking and analytical problem-solving abilities
- Ability to work collaboratively in a fast-paced, team-oriented environment
