We are seeking a dedicated and experienced SUD Case Manager to join our team. The ideal candidate will have substantial experience working with higher acuity behavioral health clients and possess psycho pharmaceutical expertise. This role is crucial in supporting our clients through personalized care plans and ensuring they receive the necessary resources and services for their mental health and well-being.
Key Responsibilities:
- Conduct comprehensive assessments of clients' needs, strengths, and preferences.
- Develop and implement individualized care plans.
- Coordinate with healthcare providers, family members, and community resources to support clients' treatment goals.
- Monitor clients' progress and adjust care plans as necessary.
- Provide psycho pharmaceutical support and guidance as needed.
- Maintain accurate and up-to-date documentation of all client interactions and progress.
- Advocate for clients' needs and rights within the healthcare system.
- Ensure compliance with state regulations and organizational policies.
Qualifications:
- Minimum of 1 years of experience working with higher acuity behavioral health clients.
- Experience with substance use disorder and mental health treatment plans.
- Strong organizational, communication, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic approach to client care.
- Deep understanding of the 12 steps and recovery
